Your insurance company assigns a set amount for payment of services to your physician. The co-pay is what your insurance company has assigned as your responsibility to pay at the time of service as part of the total amount due. The insurance company deducts the co-pay amount from the payment paid to the physician. Insurance companies require that physicians collect the co-pay at the time of service and at each visit.
Every insurance plan is different. Most insurance cards list co-pay amounts on the front of the card. Most co-pays differ by primary or specialist physicians. We are specialists. Your insurance plan booklet will list co-pay amounts, and you can always call the customer service number on the back of the insurance card for this information.
Some insurance plans have shifted some of the financial burden to the patient. This means you will pay for procedures, or even the visit itself, up to a pre-determined dollar amount. Deductibles range from zero to thousands of dollars and are determined by the employer and insurance company. The deductible amount is non-negotiable, and means that you will pay for all procedures and surgeries up to that amount at our contracted rate with your insurance company. Some insurance plans require the patient to pay for office visits, pathology, and laboratory studies up to the deductible amount. You can always call the customer service number on the back of your insurance card if you have any questions regarding your benefits.
It is our policy that you pay for your visit at the time of service. If you have a deductible, co-insurance or cost share plan we do require a payment. We have individual contracts with your insurance plan that inform us of your discounted rate per each visit that goes towards your deductible, or co-insurance. We accept Cash, Check, Debit, Visa, MasterCard, Discover and American Express.
In the state of Florida you may see a dermatologist up to 5 times a year without a referral. However, we do ask that if your primary care physician did refer you that you let us know so that we can send them our findings during your visit with Dr. Mahoney. If they have an area of concern, we want to inform the primary care physician. It is prudent to make your primary care physician aware of any dermatologic findings; this improves communication and continuity of care. If your insurance policy requires that you obtain a referral and or authorization, it is your responsibility to contact your primary care physician prior to your appointment and have it faxed to the office.
Your insurance card is required at each visit. You cannot be seen without it. If you have not been sent one from your insurance company, you can print a temporary card off their website, or call the customer service number listed in your booklet or on their website. Please also bring the following: –
- List of medications
- Name and phone number of preferred pharmacy that you wish to have your medications sent
- Insurance card
- Photo ID
We make every effort to be timely with the appointment we have scheduled for you and it is a priority! When we do occasionally run behind we do ask for your understanding. We will inform you if the physician is running behind. To assist with this we do ask that patients that are more than 15 minutes late must be rescheduled. Since we do not double book our appointments we do ask that you kindly give us 24 hours notice to cancel. We do offer a variety of services to remind you of your appointment such as email, text and phone calls as a courtesy. There is a $50.00 fee for appointments cancelled without given notice. There is a $100.00 fee for surgery appointments cancelled without given notice.

The office hours are: Monday, Wednesday, Thursday, Friday from 8:00 am to 5:00 pm. On Tuesdays we are open 8:00 am to 4:40 pm.
